Keys to Health

Keys to Health is a year-long program that promotes realistic lifestyle changes, emphasizes weight loss by increasing physical activity, healthy eating and behavior modification. The program is ideal for adults at risk for type 2 diabetes and offers assistance with reducing their risk for developing the disease through making changes that will improve their overall health and well-being.
Sessions are led by a certified lifestyle coach to empower participants to take charge of their health and focus on lasting results.
What will I learn from Keys to Health?
Keys to Health is a free program for Sharp Community Medical Group members that offers education on:

- Sustaining lifestyle changes
- Peer support
- Stress management
How can I join Keys to Health?
Keys to Health is currently servicing a pre-selected population of Sharp Health Plan members. To see if you qualify call Sharp Community at 858-499-3040.
How long is the Keys to Health program?
This program consists of 25 learning sessions that are available in person or online through web-based learning modules. The first 16 sessions participants will attend weekly, then two sessions bi-weekly and one session monthly for the last seven sessions of the program.
