The Outpatient Anticoagulation Program can help you and your Sharp Community doctor in the management of anticoagulation therapy to get the best results for your health and well-being. Anticoagulants, also called blood thinners, are medicines that prevent blood clots from forming or from getting larger. Clots that form in your heart, arteries or veins can cause heart attack, stroke or blockages.In partnership with your doctor, Sharp Community clinical pharmacists may assist by:
- assessing and adjusting medicine regimens and dosages
- authorizing refills
- providing you with patient education
- ordering laboratory tests, as appropriate, for medication therapy
- helping you take your medicine as prescribed to reduce adverse effects and drug reactions
- coordinating perioperative bridging
