Sharp Community Medical Group is dedicated to fulfilling your wellness needs. With our Case Management services, our skilled team works with you and, as appropriate, your family, your primary care doctor and other health care resources to guide you on your journey to best health.
Case Management programs
Everyone deserves the opportunity to be in good health. At Sharp Community, we offer our members more choices to assist you on your wellness journey. Our Case Management Resources Education (CARE) Program can help you along the way.
The CARE Program supports our members by providing information and resources to meet a variety of needs that may adversely affect your health and well-being.
Through the CARE Program a Case Manager may assist with the following:
Transportation for medical care
Access to adult day health care, social day care or legal services
Application for public benefits
Location of support or educational groups
Funding assistance for medication or non-covered equipment
Food resources
Some health conditions require close attention and a more intensive coordination of services. Complex Case Management may be a consideration for Sharp Community members who are at risk for:
Hospital admission or readmission
Treatment complications
Need for high-cost medical services
Care coordination for a serious illness or injury
Potential need for community resources
One of our Case Managers works with you, your Sharp Community primary care doctor and designated specialists, if applicable, to develop a collaborative approach to your care. This collaboration with your doctors is an important component of your Case Manager's role and helps address specific health needs through care planning, communication and coordination.
Together, you and your Case Manager will develop a personal plan that will promote self-reliance and improve your quality of life with an expectation of reducing the need for hospitalization and frequent use of the emergency room for routine care.
Self-measuring is one of the easiest ways to be proactive about your health and reach your blood pressure goals. High blood pressure increases the risk of having a heart attack or stroke. It can also increase risk of heart failure and kidney disease.
Regular checks with your blood pressure monitor and tracking your results can help you succeed in lowering your numbers. Controlling blood pressure is key in lowering your risk for heart disease, kidney failure, stroke, and other health problems.

Need help with measuring your blood pressure?
If you have high blood pressure, the Medication Therapy Management (MTM) program may work with you and your doctor to help control your blood pressure. For tips on the best way to measure your blood pressure and to ensure proper cuff placement, call our MTM team at 858-499-4582.
Our Case Managers can also help you stay on top of your blood pressure through health and wellness coaching through our Adult Disease Management program. They can work with you to discuss lifestyle modifications to improve blood pressure control.
Get the best results from your medications. The medicine prescribed by your Sharp Community doctor is important in staying healthy. Understanding your medications and taking them safely is key. The Medication Therapy Management (MTM) Program supports you to be an active participant in your health care, enabling you to be more knowledgeable and responsible in your medication use.
The MTM team includes Sharp Community clinical pharmacists and board-certified pharmacy technicians who work with you and your doctor to review medication regimens for appropriateness and provide patient education.
Managing diabetes can be complicated and if left unmanaged can cause serious health issues. With the myAgileLife program, Sharp Community members with type 2 diabetes have access to this health coaching and support text messaging tool that can help you:
Choose healthier foods
Get and stay active
Stay on track with taking medicines
Keep up with your recommended health screenings
Achieve your health goals
Better manage your diabetes
Take control of your health, one text at a time.
myAgileLife offers daily text messaging support and encouragement over a six-month period. It is designed to help people who are diagnosed with diabetes master the foundational behaviors to successfully maintain control of their health.
Members enrolled in the program receive three scheduled messages per day, including important topics such as monitoring blood glucose levels, heart health as it relates to diabetes and more.
You can also add up to five personal self-care reminders to take your medicine, check your blood glucose and get physically active.
Unlimited, interactive text messaging support is also available whenever you have a question or concern.
The Outpatient Anticoagulation Program can help you and your Sharp Community doctor in the management of anticoagulation therapy to get the best results for your health and well-being. Anticoagulants, also called blood thinners, are medicines that prevent blood clots from forming or from getting larger. Clots that form in your heart, arteries or veins can cause heart attack, stroke or blockages.
In partnership with your doctor, Sharp Community clinical pharmacists may assist by:
Assessing and adjusting medicine regimens and dosages
Authorizing refills
Providing you with patient education
Ordering laboratory tests, as appropriate, for medication therapy
Helping you take your medicine as prescribed to reduce adverse effects and drug reactions
Coordinating perioperative bridging
At Sharp Community Medical Group, we understand that moving back home from a hospital or skilled nursing facility can be difficult. Our expert Case Managers with Transitional Care Management are here to help smooth that transition and guide you to recovery.
Shortly after you leave an acute care facility, a registered nurse Case Manager will contact you by phone or through face-to-face telehealth video visit to ensure you have the support and information you need to settle back home and return to your way of life. Our check in with you post discharge is to help:
Identify and coordinate treatment plan concerns related to your medication regimen
Get you back to your daily activity levels and independence
Prevent unplanned hospital readmission
Having personalized care you can count on is important as you heal. Within several days of discharge, your Case Manager will coordinate a visit with your Sharp Community doctor ensuring your continuity of quality care.
We provide personalized care before, during and after your transplant surgery. Getting a transplant procedure can raise a lot of questions and concerns. Our Transplant Case Management team is here to help you and your loved ones throughout the entire process. From the time you speak to a Sharp Community-affiliated specialist for a possible transplant to post-surgical recovery, we provide the compassionate and expert care you need for long-term success.
An organ or stem cell transplant is a major procedure and can take both a physical and mental toll on your health. With telehealth video or phone visits, your registered nurse case manager is with you every step of the way.

Talk to your Sharp Community primary care doctor
You can also talk to your primary care doctor to see if you qualify for Case Management services. Your primary care doctor can send us a referral and our nurses will review your health history.
Understanding your health history is key in helping you feel your best. In addition to your case manager, your care team may include your Sharp Community primary care doctor as well as a multi-disciplinary team of nurses, pharmacists, care coordinators and social workers. This partnership is further strengthened with the support of your family and empowering them with the knowledge to help you stay healthy.
